As reported by Bollywood Hungama, the censor board asked the makers to modify several words and scenes before granting approval to the film. The word ‘item’ was replaced with ‘Kumari’, one term was muted and an “inappropriate” word was removed from a few other sequences. These changes were implemented to meet the certification guidelines. Interestingly, the CBFC did not demand the removal of any romantic or intimate scenes but paid particular attention to the film’s violent content.

Reports say that the censor board asked for several changes to reduce the film’s violent scenes. A throat-cutting shot was shortened by about three seconds, and two quick cuts were made to scenes showing blood on a woman’s face and a head injury. A moment where a woman gets slapped was also trimmed. The film now includes anti-smoking and anti-tobacco messages, and the disclaimers and voice-over have been made about two minutes longer.

After implementing the changes suggested by the CBFC, ‘O'Romeo’ received official approval with a runtime of 178.41 minutes, which equals nearly two hours and 58 minutes. The certification was granted on February 11, clearing the path for the film’s theatrical release on February 12, 2027, coinciding with Valentine’s Day weekend.
