45 follows the journey of Vinay (Raj B. Shetty), a carefree software engineer living a content life with his mother and girlfriend Meghana. His world turns upside down after he accidentally runs over a black dog named Rosie, dearly loved by local don Rayappa (Upendra). Instead of immediate retribution, Rayappa gives Vinay 45 days to reflect on his actions, invoking the Garuda Purana’s belief about life after death.

Haunted by guilt and fear, Vinay begins experiencing chilling dreams that mysteriously start manifesting in reality — including premonitions of his own fate. Amid this psychological and emotional turmoil, he encounters Shivanna (Shiva Rajkumar), a compassionate yet powerful figure who becomes his guide and protector. As the countdown begins, Vinay is forced to confront his inner demons, karmic consequences and a destiny that seems beyond his control.
