Probe finds chain of lapses in Keesara toll plaza crash
Vijayawada: The investigation into the fatal accident at Keesara toll plaza near Kanchikacherla on the Vijayawada-Hyderabad NH has revealed a sequence of events and possible lapses that led to the death of senior advocate Srinivasa Rao.According to the police, the incident unfolded within seconds. Srinivasa Rao's car reportedly shifted from lane 3 to lane 2 of the toll plaza while approaching the toll booth. At the same time, a heavily loaded fly ash tanker entered the plaza at high speed. Unable to slow down, the tanker driver lost control, rammed into the divider, and overturned, crushing the car in lane 2.Overspeeding is said to be the primary cause of the crash. The tanker driver, who later surrendered, told police he sensed danger moments the before impact but could not control the vehicle due to its heavy load. The tanker eventually toppled on the car, crushing it completely. CCTV footage shows the tanker speeding into the plaza and hitting the vehicle within moments.Police have registered a case based on a complaint filed by the deceased advocate's brother, Sudhakar, naming the tanker driver for negligence. However, officials are also examining the role of the toll plaza management. Preliminary findings indicate the absence of adequate warning systems such as speed breakers, signboards, and speed control measures at the toll point.
Further complications arose during rescue operations as the tanker, filled with ash, blocked multiple lanes. Authorities said unloading the material posed risks, as releasing it could spread ash across the toll plaza. Arrangements are being made to transfer the load into another tanker using compressors before removal. Officials estimate that clearing the overturned vehicle could take up to 10 hours. The probe is ongoing to determine accountability, including potential violations related to overloading and safety norms.
