Probe blames 5 junior staffers for Rajasthsn hospital death

A committee constituted to investigate the death of a 23-year-old pregnant woman at the SMS Hospital in Jaipur has acknowledged she died after being transfused with the wrong blood type and has assigned blame for the incident to five lower-level staff members, effectively exonerating the senior staff under whose supervision they worked. Despite at least three such incidents having been reported from the hospital in the past two years, no senior official has been held to account for these potentially lethal lapses.
