Essential and common tests for Tuberculosis diagnosis
Tuberculosis (TB) remains a significant global health challenge, necessitating accurate and timely diagnosis for effective management and control. A variety of tests are available for diagnosing TB, each with its own strengths and limitations. Understanding these tests is crucial for healthcare providers to ensure accurate diagnosis and appropriate treatment. Here, we delve into the essential and common tests for TB diagnosis.
TST involves injecting a small amount of purified protein derivative (PPD) tuberculin into the skin, typically on the forearm.
The test relies on the immune system's reaction to the injected substance. A positive result indicates exposure to TB bacteria but does not distinguish between latent infection and active disease.
TST is inexpensive and widely available, making it a common initial screening tool.
Interferon-Gamma
IGRAs, such as the QuantiFERON-TB Gold test and the T-SPOT.TB test, measure the release of interferon-gamma by T-cells in response to TB antigens.
Unlike TST, IGRAs are not affected by prior BCG vaccination and require only one patient visit for blood collection.
IGRAs offer improved specificity compared to TST, particularly in populations where BCG vaccination is common.
Sputum Smear.
Sputum smear microscopy remains the most widely used test for diagnosing pulmonary TB in resource-limited settings.
This test involves staining sputum samples with special dyes to detect acid-fast bacilli (AFB) under a microscope.
While rapid and inexpensive, sputum smear microscopy has limited sensitivity, especially in patients with paucibacillary disease.
Sputum culture involves inoculating sputum samples onto culture media to allow the growth of Mycobacterium tuberculosis bacteria.
Culture is the gold standard for TB diagnosis, offering high sensitivity and specificity.
However, culture results may take weeks to obtain, delaying diagnosis and treatment initiation.
Chest X-ray is essential for diagnosing pulmonary TB and assessing disease severity.
Typical findings include infiltrates, cavitations, and hilar lymphadenopathy.
While not definitive on its own, chest X-ray complements other diagnostic tests and aids in treatment monitoring.
Nucleic Acid Amplification Tests.
NAATs, such as the Xpert MTB/RIF assay, detect the genetic material of TB bacteria directly from sputum samples.
These tests offer rapid and accurate diagnosis, with results available within hours.
NAATs are particularly useful in diagnosing drug-resistant TB and in HIV-infected individuals.
Drug Susceptibility
In extrapulmonary TB or cases where pulmonary TB diagnosis remains uncertain, biopsy and histopathological examination of affected tissues may be necessary.
Histopathology can reveal granulomas and caseous necrosis characteristic of TB infection.
