Why straight lines start looking wavy after 50: Doctor explains what Age-Related Macular Degeneration (AMD) really feels like
Age-Related Macular Degeneration (AMD) doesn’t get talked about enough in India, and that’s a big problem. One reason is simple: most people think vision loss is just a “normal part of ageing.” If someone over 60 says they can’t read clearly or need brighter light, families often shrug it off instead of getting an eye check-up.Data suggests AMD affects around 1–3% of India’s overall population, but the numbers rise sharply with age, studies show roughly 7–10% of people over 60 may have some form of AMD. With India’s elderly population growing fast, that translates into millions at risk. Yet awareness is still low compared to cataract or glaucoma. Why? AMD, especially the dry form, progresses slowly and doesn’t have a “quick fix,” so it doesn’t get the same public attention. In rural areas, limited access to retina specialists and low routine screening rates make early detection even harder. Add to that low health literacy and the fact that early AMD doesn’t cause pain, and it quietly slips under the radar. The result? Many patients only seek help when central vision is already badly affected.
AMD isn’t rare, it’s just rarely recognised in time.“Age-Related Macular Degeneration (AMD) is becoming a major public health concern in India. AMD involves the centre of the retina, which has maximum concentration special cells called cone cells. This central area is important for reading, writing, focusing, and is important for central vision. AMD is traditionally seen in people over 55. AMD affects around 2% to 4 % of the population above 50 years of age , and around 40% above 70 years of age. With the rising life expectancy, urban lifestyles, and environmental stressors, AMD is expected to grow further,” Dr. Aditya Kelkar, Director, NIO Super Speciality Hospital, Pune told TOI Health.260 million Indians consume tobacco, 14–16% drink alcohol: The preventable crisis behind over 1.3 million deaths in the country

Causes, symptoms

The expert says, AMD occurs in individuals when the central part of the retina called macula, which is responsible for sharp vision, starts to deteriorate. Other causes of AMD include aging, genetic predisposition, smoking, poor diet, obesity, hypertension, screen exposure, and prolonged exposure to UV rays . The degeneration is of two types Dry Degeneration and Wet Degeneration. Dry Degeneration is a common form which is relatively mild, causing slight reduction in reading and writing vision. This type of degeneration proceeds extremely slowly as against the Wet type of degeneration that leads to rapid vision loss. Earliest symptoms could be reduced vision or distorted vision, waviness, black spots in central vision. , difficulty in reading and writing, and hard to recognize faces etc. Because these signs are easily overlooked due to many reasons, AMD is diagnosed late. Dr. Aditya recommends the following preventive measures:
  • Regular eye check-ups
  • Make lifestyle changes by preferring healthy diet
  • Avoid smoking
  • Manage blood pressure
  • Reducing screen time and protecting eyes from UV exposure
Dr Kelkar also states that, while AMD cannot be fully cured, modern treatments can slow progression and preserve vision. For dry degeneration, doctors may consider giving anti oxidants and low vision aids such as magnifiers if required. For wet degeneration, we can control it with the regular use of Anti-VEGF injections. These injections stop the growth of abnormal blood vessels under the retina, thereby reducing the leakage and swelling of the central part of the retina. And these injections might have to be continued over a prolonged period of time. Now there are more than six or seven varieties of drugs available in this category. There are also special home tests called Amsler grid , the doctors can do these specialised tests in OPD as well for diagnosing and monitoring this condition. Optical Coherence Tomography (OCT) , is a quick, painless and non-invasive test that is critical for diagnosis and management of these conditions. It provides very high resolution retina scans which will enable earlier detection as well.
