Balakrishna was born in Madras to Telugu actor and former Chief Minister of Andhra Pradesh, N. T. Rama Rao, and his wife Basavatarakam. Growing up in Madras, he was exposed to the Telugu film industry, which eventually led him to pursue a career in acting. At the age of 14, he made his debut as a child artist in the film 'Tatamma Kala' in 1974, directed by his father.

Balakrishna's acting career spans over 100 Telugu films, where he showcased his talent and versatility in various roles. In the 1980s, he gained commercial success with films like 'Sahasame Jeevitham' (1984), 'Mangammagari Manavadu" (1984), 'Apoorva Sahodarulu' (1986), and 'Muvva Gopaludu' (1987), among others. He has received three Nandi Awards for Best Actor for his outstanding work in 'Narasimha Naidu' (2001), 'Simha' (2010), and 'Legend' (2014).
