Josh Allen Injury Update: Star QB faces uncertainty as the Bills reveal possible surgery and offensive issues
This season was meant to be a quiet reset for the Buffalo Bills. But it went the opposite way. The focus should have been on regrouping and moving forward. Instead, the conversation snapped right back to the same uncomfortable place Buffalo knows too well. Everything still revolves around Josh Allen.
Two concerns sat side by side. Allen may need surgery on his right foot. And the O-line around him feels thinner and more unsettled than it should for a team chasing a Super Bowl. All together, it creates real questions that Buffalo cannot brush aside.
“He could have to have something done to his foot, potentially here soon,” Beane said during the press conference. “But it would not impact OTA stuff if he does.”

WR Keon Coleman remains a major question mark after owner Terry Pegula’s comments during a 53-minute press conference on Jan 22, 2026. Veteran WR Brandin Cooks addressed the situation the following day.
“I think this guy can play at a high level,” Cooks said during a locker room media session at One Bills Drive. “He’s misunderstood. He’s working. He’s becoming a pro.”
Coleman’s stats did not reflect a second-year leap. He finished the season with 38 catches for 404 yards and four TDs in 13 games. Buffs expected more, but it did not happen.
Later, after the matchup, Josh Allen broke down in tears on Saturday: “I let my teammates down tonight,” Allen said postgame on Saturday. … “Just missed opportunities throughout the game. It’s been a long season. I hate how it ended. It’s gonna stick with me for a long time.”
Every road leads back to Allen. Buffalo needs him healthy before anything else. The Bills also need to give him an offense that does not ask him to be perfect every week. Surgery may solve the foot issue. Roster changes may address the offense. Until then, the franchise enters another offseason with its most important player carrying the heaviest load.
