Imagine feeling tired as if you’ve run a marathon — every single morning — but you haven’t even left your bed! Or maybe your hands and feet keep tingling like tiny pin-pricks, but you shrug it off as “just nerves.”

Sounds exhausting and ridiculous, right?

However, these are not always minor annoyances to brush under the carpet — they might be signs that your body is running low on vitamin B12.

Think of vitamin B12 as the quiet stage manager of your body’s show: not glamorous, but without it, the performance stumbles!

Vitamin B12 is quietly essential: it helps make healthy red blood cells, supports nerve function, and powers up your brain. But the tricky part is, when B12 levels drop, the symptoms often come on so slowly and subtly that most people don’t notice anything wrong — until things start to feel more serious. Researchers warn that B12 deficiency is more common than you might think, especially among older adults, vegetarians or vegans, and people on certain medications. And because the warning signs are vague, many keep going about life, tired and foggy-headed, not realizing that a simple blood test (or, in some cases, a supplement) could change everything.

Persistent fatigue

Feeling exhausted even after a weekend of decent sleep? B12 helps your body make healthy red blood cells that carry oxygen. Without enough, you can feel weak, breathless, or unusually tired from small tasks. Because this develops slowly, it’s easy to blame stress or poor coffee.

Pins-and-needles, numbness, or poor balance

Tingling or numb hands and feet, clumsiness, or trouble walking steadily are red flags. B12 is crucial for the protective coating (myelin) around nerves — when it’s low, nerve signals become fuzzy. If left untreated, nerve damage can become long-term. So, it’s wiser not to ignore the newly crept up or already worsening numbness.

Low B12 can cause forgetfulness, poor concentration, slowed thinking, mood swings, and depression. Severe or long-standing deficiency may even cause confusion and mimic dementia. Who’s most at risk?

Older adults, as their stomach acid and absorption tend to fall with age, are more prone to falling prey to B12 deficiency. Those with autoimmune pernicious anemia, GI surgery, Crohn’s disease, or other absorption problems are also vulnerable. People on long-term metformin or proton pump inhibitors (PPIs) are an easy target, as these common drugs can reduce B12 absorption.

Interestingly, vegans and strict vegetarians are prone to suffering from B12 deficiency, because natural B12 sources are mainly animal foods, and their diet is devoid of those foods. For them, fortified foods or supplements are essential to meet the B12 needs.

Why most people miss the signs

The liver is a B12 piggy bank: The body stores large B12 reserves in the liver that can last years, so deficiency often develops slowly. That long lag makes early symptoms subtle and easy to ignore.

Symptoms are common and non-specific: Fatigue, mild mood changes, and occasional numbness are common complaints with many causes. Busy lives, poor sleep, or other illnesses are usual and frequent suspects — so a case of B12 deficiency can be missed unless someone pays enough attention to check.

The loophole in science

Folate and masking: Folate (vitamin B9) in food or supplements can correct the anemia caused by B12 deficiency without fixing nerve damage, which can hide the problem until neurological symptoms appear. That’s why relying only on red blood cell counts can be somewhat misleading.

Standard blood tests can be misleading: A basic serum B12 level is a good starting point, but not perfect. Some people have “borderline” results yet have a cell-level deficiency. Functional tests — such as methylmalonic acid (MMA) and homocysteine — can reveal deficiency even when serum B12 looks okay. If symptoms don’t match the basic test, doctors may order MMA or homocysteine next for further examination.
