Heart health: That leg pain when you walk isn’t always old age; it might be peripheral artery disease (PAD)

It starts with leg pain… then turns into a silent heart threat

If your legs ache, cramp, or feel tight after walking just a few blocks, and you keep blaming it on “getting older”, you might be missing a serious health red flag. That leg pain when walking could be your body’s way of saying your blood flow is in trouble. Experts warn it may not be simple aging or muscle fatigue at all, it could be Peripheral Artery Disease (PAD), a common but often overlooked circulation problem that increases your risk of heart attack and stroke.

What is peripheral artery disease (PAD)?

Peripheral Artery Disease happens when fatty plaque (a mix of fat, cholesterol, and other substances) builds up inside your arteries, the same arteries that carry oxygen-rich blood to your legs and feet. Over time, those arteries become narrowed or blocked, cutting off circulation to your lower body. The result? Your leg muscles don’t get enough oxygen when you’re active, leading to pain, cramping, or heaviness in your legs, especially while walking or climbing stairs.
According to the American Heart Association, more than 8.5 million Americans live with PAD, and the CDC reports that many people don’t even know they have it until the pain starts to interfere with daily activities.

Why that “getting older” leg pain could be something more serious

The tell-tale symptom of PAD is leg pain when walking that goes away with rest, what doctors call intermittent claudication. It happens because your muscles are demanding oxygen during activity, but your arteries can’t deliver enough blood.

Unlike arthritis or general stiffness, PAD pain typically appears after a predictable amount of walking and eases up after you stop. If blood flow continues to worsen, that pain might start showing up even when you’re resting or lying down.
Other warning signs of poor circulation and PAD include:
Numbness or weakness in your legs
Cold feet or lower legs (one colder than the other)
Sores or wounds on your toes or legs that heal slowly
Shiny skin or hair loss on your legs
Weak or absent pulse in your feet
Ignoring these signs can lead to serious complications, including tissue death (gangrene), amputation, or even heart attacks and strokes.

Who’s most at risk for PAD?

PAD is caused by atherosclerosis, the same plaque buildup that leads to heart disease. Risk factors include:
Smoking: Smokers are up to four times more likely to develop PAD.
Diabetes: High blood sugar damages blood vessels, speeding up plaque formation.
High blood pressure and high cholesterol: Both make artery narrowing worse.
Age: Risk jumps after age 50.
Family history, obesity, and inactivity also add to the risk.

Why PAD is more than just leg pain

Here’s the scary truth: Peripheral Artery Disease is a warning sign of widespread artery damage. The same plaque blocking blood flow to your legs is often clogging arteries in your heart and brain too.
According to Harvard Medical School, having PAD doubles your risk of a heart attack or stroke. Left untreated, severe PAD can turn into critical limb ischemia — where blood flow becomes so restricted that tissue begins to die. That’s when doctors are racing to save the limb.

How to treat and manage PAD

PAD can often be managed and sometimes even improved with early treatment and lifestyle changes. The main goal is to restore blood flow and prevent blockages from getting worse.
Here’s what doctors typically recommend:
Quit smoking: This is the single best thing you can do. Circulation improves almost immediately.
Exercise regularly: Walking programs supervised by doctors can dramatically reduce leg pain and improve blood flow.
Control chronic conditions: Keep your blood pressure, cholesterol, and blood sugar in check.
Take prescribed medications: Your doctor may recommend blood thinners, cholesterol-lowering drugs, or circulation-boosting medications.
Consider procedures: In severe cases, angioplasty or bypass surgery can reopen blocked arteries and restore blood flow.
Even simple habits, like eating heart-healthy foods, staying active, and losing weight, can help your arteries stay open longer.

7/9

When to see a doctor about leg pain

If you notice leg cramps, heaviness, or pain when walking, don’t just chalk it up to age. Especially if you smoke, have diabetes, or a family history of heart disease, see your doctor. That “just aging” pain might be your body’s way of sounding the alarm.
Peripheral Artery Disease is treatable, but only if you catch it early. While aging is inevitable, poor circulation isn’t. The next time your legs ache after a walk, don’t ignore it, it could be your heart trying to get your attention.

Disclaimer: This content is for informational purposes only and is not a substitute for professional medical advice. Leg pain while walking can have multiple causes, including peripheral artery disease (PAD). Always consult a qualified healthcare provider for an accurate diagnosis and appropriate treatment if you experience persistent or severe symptoms.

9/9

Early symptoms of PAD

Leg pain while walking (claudication): Cramping, fatigue, or pain in the legs, hips, or buttocks during activity that goes away with rest.

Numbness or weakness in the legs: Often felt during movement or after prolonged periods of walking.

Coldness in the lower leg or foot: Especially when compared to the other leg.
