Fatty liver abdominal pain vs stomach pain: How to tell the difference

Stomachache isn’t something uncommon in our day-to-day lives. Everyone gets belly pain once in a while – sometimes after a heavy meal, sometimes eating something that didn’t agree with you, or sometimes even from stress and anxiety. You might have wondered whether it’s due to indigestion, gastric upset, ulcers, acid reflux, or something more serious.

Most times, it’s just a regular stomachache, indigestion, or gas. But sometimes, pain in the abdomen (belly) isn’t from the stomach at all.

It may come from another organ, such as the liver. The one condition that can cause upper-abdominal discomfort is fatty liver disease, which is becoming more common worldwide. Fatty liver (particularly non-alcoholic fatty liver disease, NAFLD or the newer term MASLD) often hides quietly, without obvious symptoms. But when it does cause pain, many people mistake it for regular stomach pain or digestive problems.

Fatty liver happens when fat builds up inside liver cells. It is often silent, with no loud enough symptoms, for a long time. But when symptoms appear, abdominal discomfort may be one of them. Understanding the differences between fatty liver pain and stomach pain matters because misinterpretation can delay correct diagnosis and proper treatment. As they come from different organs, they have different warning signs and require different treatments.

What is fatty liver disease

Fatty liver disease refers to fat accumulation in the liver beyond what is normal. There are two main types: nonalcoholic fatty liver disease (NAFLD) and alcoholic fatty liver disease (AFLD). Factors such as obesity, high cholesterol, type 2 diabetes, poor diet, and excessive alcohol use increase risk. Most people with fatty liver have no symptoms until the disease progresses.

What is a regular stomachache

Stomach pain is a broad term people use when they feel pain or discomfort in the upper central abdomen (often left of the midline, under the ribs). It can come from the stomach itself (gastritis, ulcers) or from nearby organs (esophagus, small intestine).

Fatty liver pain or discomfort is often felt in the upper right side of the abdomen (just under the ribs). It may also spread toward the back or right shoulder in some cases.

Whereas, stomach pain usually appears in the upper central abdomen (epigastric region) or slightly to the left, under the rib cage. It may also cause discomfort in the stomach area that moves after eating.

For fatty liver, the pain is usually a dull, achy, or heavy feeling rather than sharp stabbing. It might increase when the liver is under strain, after eating fatty food, or if alcohol is consumed.

Meanwhile, stomach pain is often sharp, burning, cramp-like, or gnawing pain. Common triggers are meals (especially large, spicy, or fatty meals), lying down after eating, or certain foods. Acid reflux or gastritis often causes burning or soreness.

Fatty liver pain is more likely to be persistent or recurrent, not just occasional “stomach upset.” It may worsen over time if liver disease progresses.

In case of regular stomach pain, it’s episodic, often acute flares (e.g., after spicy food or stress). It may go away with treatment or a change in diet.

The pain stemming from fatty liver may be more noticeable after big or fatty meals, or when the liver is stressed. It may be more constant or recurring, less tied to immediate food intake. However, the early stages may be silent.

On the other hand, regular stomach pain is often closely tied to meals: before eating, after eating, when the stomach is empty. It may worsen or improve quickly depending on eating, antacids, etc.

Along with pain, people suffering from fatty liver may notice fatigue, weakness, nausea, loss of appetite, bloating or feeling of fullness, and in some cases jaundice (yellowing of skin/eyes), dark urine, or swelling in the abdomen.

In case of regular stomach pain, the associated symptoms may include heartburn, belching, indigestion, bloating, a feeling of fullness quickly after eating, nausea, or vomiting. Relief may come from antacids or avoiding certain foods.
