Fatty liver disease is easy to miss because it rarely knocks at the door with dramatic symptoms. In the United States, “an estimated quarter of people” are thought to have nonalcoholic fatty liver disease (NAFLD), and global reviews put adult prevalence in the tens of percent range, a quiet epidemic that often only shows itself through small changes on the surface of the body.

For many people, the first clear clues arrive in the mirror: a yellowed eye, a persistent flush, a web of tiny blood vessels. Those signs deserve attention because they can point to a problem that’s preventable or manageable when found early.

Jaundice

The most unmistakable facial sign is jaundice: a yellowing of the sclera and skin caused by excess bilirubin in the blood. Bilirubin is a pigment produced when red blood cells are broken down; a healthy liver clears it. When the liver can’t keep up, the color first shows in the eyes and face before it becomes widespread. Clinicians treat visible jaundice as a major warning sign that liver function needs immediate evaluation, and major clinical guides and patient resources recommend prompt medical assessment rather than cosmetic fixes.

Spider Angiomas

Those delicate, spider-like red marks that sometimes appear on the face or neck are more than cosmetic. Known as spider angiomas or spider nevi, they blanch when you press them and then refill from the center, a physical pattern doctors associate with hormonal and vascular changes that occur when liver function declines. Multiple spider angiomas are commonly reported in patients with cirrhosis and have been studied as correlates of liver fibrosis and deteriorating liver function. Spotting several of them, especially alongside other symptoms, should prompt a clinician’s appraisal.

Rosacea

Persistent facial redness, visible tiny vessels, and periodic bumps are classic rosacea symptoms. Emerging dermatology research has explored a link between rosacea and metabolic liver conditions like NAFLD; while rosacea is not a direct diagnosis of liver disease, several experts have argued that midface vascular changes may serve as a visible biomarker pointing to deeper metabolic or inflammatory processes. That doesn’t mean everyone with rosacea has liver disease, but the overlap is strong enough that dermatologists sometimes suggest screening for metabolic risk factors when facial redness is unexplained or new in midlife.

Palmar Erythema

Changes that begin in the hands can be telling for the whole body. Palmar erythema — a symmetric redness of the thenar and hypothenar eminences of the palms (sometimes called “liver palms” or “paper-money skin”) — has been noted repeatedly in reviews of liver disease skin signs. It is non-specific and can occur for other reasons (pregnancy, autoimmune disease), but it is also a classic sign in patients with chronic liver disease: clinicians and reviews report it commonly in cirrhosis and other advanced liver conditions, and it often appears alongside other hepatic stigmata.

Pruritus

A deep, persistent itch with little or no rash can be one of the hardest signals to interpret. In cholestatic or bile-flow disorders and other liver conditions, patients commonly report intense pruritus long before other dramatic signs appear. Modern research implicates bile acids, lysophosphatidic acid, bilirubin, and recently identified receptors (like MRGPRX4) in the biology of cholestatic itch. Treating the symptom often helps only partially unless the underlying liver or bile issue is addressed. Because the itch can be so disruptive, clinicians treat it seriously as a potential hepatic warning sign.
